This symposium will tackle the inherent challenges in organ preservation for patients who achieve a complete clinical response (cCR). Accurate diagnostic criteria of cCR, selection of patients based on baseline features, and preferred treatment strategies will be addressed as well as the specific features of patients who achieve a "near-complete" response including the uncertainties of clear diagnostic criteria and potential risks for organ-preservation strategies for this specific group of patients. Finally, we will address the risk for subsequent distant metastases after achieving a cCR with or without the development of local regrowth.
