Purpose/Background: Colonic volvulus, a condition where the large bowel twists around its mesentery, most commonly occurs at either the cecum or sigmoid colon. The splenic flexure has four attachment sites, thus splenic flexure volvulus is a rare entity. There are approximately six reported instances in the literature of splenic flexure volvulus, most commonly associated with chronic constipation secondary to congenital anomalies, psychiatric history, or previous abdominal surgery. Volvulus is a clinically significant condition as it can lead to bowel obstruction or ischemia, presenting an acute surgical emergency.Methods/Interventions: Here, we present the case of a 67 year-old female who presented to the emergency department with abdominal pain and diet intolerance on postoperative day ten following elective robotic sigmoid colectomy for past history of diverticulitis. Workup included computed tomography imaging which demonstrated colonic volvulus at the splenic flexure, prompting an emergent surgical intervention.Results/Outcomes: Due to prompt identification, the patient had viable bowel on intraoperative colonoscopy following laparoscopic splenic flexure detorsion, and thus was spared from further colonic resection. A colopexy alone was performed and the patient had an uneventful postoperative course.Conclusion/

Discussion: Thus, colopexy appears to be a viable option for early volvulus definitive treatment."CT At Time of ED Presentation
