eP186 - Association Between Tumor Genetic Profiles in Colorectal Cancer and Initial Sites of Metastasis

Purpose/Background: Tumor genetics play a pivotal role in colorectal cancer, influencing cell proliferation and tumor formation. The relationship between specific genetic variations and the sites of initial metastasis is closed linked. This study aims to investigate whether there is a discernible association between the tumor genetic profiles of colorectal cancer and the sites of initial metastasis.Methods/Interventions: This retrospective study collected data from a single medical center in Taiwan between June 2019 and October 2023. It included patients with histologically proven adenocarcinoma and metastatic colorectal cancer who had undergone Next-Generation Sequencing (NGS) testing on tissue from the primary tumor site. Exclusion criteria encompassed non-metastatic colorectal cancer and patients without NGS or comprehensive genomic testing, as well as those with local surgical margin recurrence. Clinical data, including patient characteristics, tumor location, and the initial site of metastasis, were analyzed to explore the potential association between tumor genetic profiles and the initial metastatic site.Results/Outcomes: The study involved 105 patients, comprising 60 males and 45 females. Among them, 73 had colon cancer, and 32 had rectal cancer. The initial metastatic sites included 52 patients with liver metastasis, 27 with lung metastasis, and 22 with peritoneal metastasis, while the remaining patients experienced metastasis to other sites, including bone and pelvic lymph nodes. The data revealed a higher prevalence of liver metastasis in patients with KRAS and APC mutations, higher rates of lung metastasis in female patients and those with NRAS mutations, and a greater occurrence of peritoneal metastasis in patients with BRAF V600E mutations. Discussion: This study demonstrates discernible associations between the genetic profiles of metastatic colorectal cancer and the sites of initial metastasis. Understanding these associations can help guide personalized treatment strategies for colorectal cancer patients.
