Purpose/Background: Colon cancer is one of the leading causes of cancer deaths around the world. There has been an uptrend in the incidence of colon cancer in the younger adult population. Prevention is paramount to improve patient outcomes and have an early detection and prompt treatment. A colonoscopy is the gold standard for the diagnosis of colon cancer. In The Dominican Republic, the current state of colorectal cancer and its screening is not well understood or documented. Our aim was to characterize the current state of screening colonoscopy in the Dominican Republic.Methods/Interventions: Retrospective review of medical records of patients that underwent Screening colonoscopy at Hospital General de la Plaza de la Salud in Santo Domingo Dominican Republic. Data extraction included age, gender, results of colonoscopy, type of polyps found if the patient had or not have advanced features of their polyps, location, pathology results regarding malignancy and adenoma, and polyp detection rate. Data analysis included descriptive statistics and differences in the incidence of polyps and malignancy based on location (proximal vs distal colon). Statistical analysis was done with SPSS statistics 29 Mac Os-Ventura.Results/Outcomes: A total of 635 records were reviewed, 75.3% of the patients were male. 71% of the patients underwent colonoscopy after age 50. 1.6% of the colonoscopy found macroscopic findings consistent with malignancy. In 64.7% of the cases, polyps were reported. 3.5% of the polyps were adenomatous, and 39.2% were hyperplastic, 1.9% of the cases were tubular adenomas.
In regards to their features, 5.8% of the polyps were considered to have advanced features (1>cm, villous features). Most of the polyps were found in the ascending colon (21.9%), followed by the rectum (18.7%). Of the polyps found: 5.4% were considered neoplastic by pathology results. There were no differences in the incidence of polyps or malignancy based on distal vs proximal location (p>0.05).Conclusion/

Discussion: Hospital General de la Plaza de la Salud saw over 50,000 patients in the age bracket recommended for screening colonoscopy. The number of colonoscopies done is below the population need. There is a high prevalence of polyps (64.7%), although most are hyperplastic. Our study identified a relatively low rate of malignancy, with only 1.6% of colonoscopies revealing findings consistent with malignancy and 5.6% of pathology results revealing neoplastic specimens, numbers similar to the USA. The mean number of polyps over 50 years of age in the United States is 52.8%, it is unknown the reason why the number is higher in the Dominican Republic. Efforts should be placed towards improving the number of patients who undergo colonoscopies in the recommended age bracket.
