Purpose/Background: Ulcerative colitis (UC) is a chronic gastrointestinal pathology characterized by chronic inflammation of the mucosa and submucosa of the colon. Active mucosal inflammation has been purposed to trigger hypersensitivity and visceral hyperactivity, leading to chronic changes in the rectal wall as fibrosis and decreased compliance which might associate with anorectal functional disorders such as incontinence, fecal urgency and tenesmus and imply significant decrease in quality of life. The objective of this study is to assess anorectal function abnormalities in UC patients and analyze their relationship with disease activity.Methods/Interventions: We performed a cross-sectional study in 15 UC patients. Demographical, clinical, biochemical, endoscopic, and histological data were collected. UC activity was assessed by Truelove-Witts Scale (TWS), Mayo Subscore (MS), Riley Histological Index (RHI) and Yamamoto-Furusho Integral Index (YFII). Anorectal manometry was recorded and performed with ManoScan 3D equipment. SPSS ver. 26 was used to analyze activity and manometry correlation or association. A p-value less than 0.05 was considered statistically significant.Results/Outcomes: The demographic, clinical and manometric characteristics of the patients are presented in table 1. Manometric parameters in UC patients showed 46.7% had hypertonia at the Internal Anal Sphincter (IAS) and 60% at the External Anal Sphincter (EAS). Additionally, 20% showed abnormal defecation maneuvers while 60% had alterations in rectal sensitivity. Two patients (13.3%) presented type I and one patient (6.7%) type II defecatory dyssynergia. Statistically significant differences were found between the extent of the disease and rectal sensitivity: first sensation (p < 0.01), urge to defecate (p < 0.02), and fecal urgency (p < 0.03). A negative correlation was observed between sensitivity parameters and disease extent: first sensation (r=-0.57, p < 0.02), urge to defecate (r=-0.59, p < 0.02), and fecal urgency (r=-0.67, p < 0.001). Statistically significant differences were also found between clinical and biochemical activity measured by ETW and the urge to defecate (p < 0.01), along with a negative correlation between these parameters (r=-0.64, p < 0.01). Differences were observed in the degree of clinical activity measured by TWS (p=0.01) and the comprehensive assessment of YFII (p=0.02), with a negative correlation between them: TWS (r=-0.74, p < 0.001) and YFII (r=-0.61, p < 0.01).Conclusion/

Discussion: UC patients exhibited significant anorectal function abnormalities, including sphincter hypertonia, rectal sensitivity alterations, and defecatory dyssynergia. The study highlighted correlations between disease extent and severity and rectal sensitivity alterations, emphasizing the importance of assessing anorectal function in UC patients to enhance their quality of life and optimize treatment.Table 1.
