Purpose/Background: Abdominal liposuction, with or without abdominoplasty, is one of the most commonly performed procedures in aesthetic plastic surgery. Reports of complications following this procedure are very rare in the literature. However, one of the rare life-threatening complications of this procedure is visceral injury and bowel perforation. This complication could be fatal and associated with difficult or late diagnosis, and surgeons must be aware of its possibility and its management.Methods/Interventions:

Case presentation: A 40-year-old female presented with acute abdominal pain associated with fever and obstipation that had developed following elective Abdominal liposuction with an abdominoplasty procedure two days previously. The procedure had been performed without any intraoperative complications.
On examination, the patient was generally looking ill, found to be hypotensive, and in moderate distress. Her abdomen was slightly distended, with generalized tenderness and visible scars from her prior abdominoplasty. Laboratory investigations showed raised inflammatory markers.
A computerized tomography scan (CT) of the abdomen and pelvis revealed pneumoperitoneum with a moderate amount of intraperitoneal free fluid. The mesenteric fat stranding was noted.Results/Outcomes: The patient’s clinical condition indicated the need for an emergency exploratory procedure with high suspicion of visceral or bowel perforation. A single jejunal perforation was found and managed with primary repair without required bowel resection or stoma diversion.Conclusion/

Discussion: The reported case shows that the diagnosis of bowel perforation following abdominal liposuction and abdominoplasty procedures is challenging, and multimodal investigations and high clinical awareness are needed. Nowadays, the procedure can be performed in the outpatient setting, possibly downplaying this potentially deadly complication that may occur. This case is one such case to be reported as jejunal perforation following this plastic surgery technique managed without bowel resection or stoma diversion.
