Purpose/Background: The SP robotic platform is a recent addition to the minimally invasive armamentarium, and as such requires investigation into its use as non inferior to traditional laparoscopic approaches. We seek to compare outcomes between cases utilizing SP robot and those using a laparoscopic approach.Methods/Interventions: A prospectively maintained database was reviewed to identify patients undergoing surgery from an SP robot and laparoscopic approach between the years 2018-2023. Patients’ demographics, clinical, disease, intraoperative and postoperative outcomes were abstracted, propensity score matched (PSM) 1:1 on baseline features and compared. The main outcome measures were the intraoperative complication, conversion rates, operative time and postoperative complications.Results/Outcomes: There were 483 patients reviewed and included (laparoscopic n=357, SP robotic n=126). The PSM yielded two well balanced cohorts, with 104 patients each. The age, body mass index, sex, previous operation rate, previous radiation rate, disease, and procedure type were similar across SP robotic and laparoscopic arms. The operative time was longer on SP robotic at mean 359 minutes (SD, 110) compared to laparoscopic at 244 minutes (SD, 124; p < 0.001). There was no conversion to open surgery on both arms. The largest incision size was similar, median 4.2cm (IQR, 1.5-5.2) for Laparoscopic and 4cm (IQR, 1.5-5) for SP robotics (P=0.7). SP robotic had shorter time to flatus (median 2 days, IQR 1-2) compared to laparoscopic (median 2 days, IQR 2-3; p=0.004). The length of stay was shorter in the SP robotics at median 4 days (IQR, 3 -5) versus laparoscopic at median 4 (IQR, 3-6). There was a lower postoperative morbidity for laparoscopy at 12.4% (n=13) for laparoscopic versus 8% (n=9) for robotic however it did not attain statistical significance (p=0.36). There were no mortalities.Conclusion/

Discussion: SP robotics offers the opportunity for a smaller overall incision size, without compromising surgical outcomes. This study demonstrated that using the SP robot is safe, with no difference in perioperative outcomes when comparted to laparoscopy.
