Purpose/Background: The oncologic risk associated with transverse colon cancer remains insufficiently understood compared to other locations, even although transverse colon has complexity of the vascular structure and proximity to other organs. This study aims to assess the oncologic significance of transverse colon involvement in patients diagnosed with stage III colon cancer.Methods/Interventions: We conducted a comprehensive analysis of 1379 patients who underwent laparoscopic surgery for stage III colon cancer between January 2003 and December 2021. Locoregional recurrence, including peritoneal seeding around the primary tumor, was a key focus. Our study involved a clinicopathologic comparison between transverse colon and non-transverse colon cancers (ascending, descending, and sigmoid), and multivariable analyses were performed to identify factors influencing locoregional recurrence.Results/Outcomes: Of the total patients, 161 (11.7%) had transverse colon cancer, while 1218 (88.3%) had cancer in other locations. No significant differences were observed in age, sex, body mass index, CEA, and T stage between the two groups. However, transverse colon cancer exhibited a higher rate of locoregional recurrence compared to other locations (8.7% vs. 3.4%, p=0.002). In the multivariable analysis, transverse colon [OR=2.209, 95% CI; 1.133-4.306, p=0.020] and advanced T stage [OR=8.452, 95% CI; 1.161-61.550, p=0.035] emerged as independent risk factors for locoregional recurrence.Conclusion/

Discussion: This study demonstrates that transverse colon cancer poses a substantial risk for locoregional recurrence in colon cancer patients. We advocate for the inclusion of specific preoperative planning for transverse colon cancer to enhance oncologic radicality and improve patient outcomes.
