eP626 - Anxiety and Depression Among Patients with Pelvic Floor Disorders and Their Correlation with Socioeconomic Status and Ethnicity

Purpose/Background: Colorectal pelvic floor dysfunctions (PFDs) negatively impact quality of life. These conditions are complex and often linked to emotional factors. Previous studies have shown anxiety and depression symptoms in PFD patients, but no research has explored their influence on the PFD care process and their relationship with Socioeconomic status (SES) and ethnicity.Methods/Interventions: A retrospective study was conducted from 2013 to 2018 at Guy’s and St Thomas’ Hospital, London UK. Patient demographics, main complaint, symptoms, psychiatric disorders, Consultant referral, biofeedback referral, diagnostics, Multidisciplinary meeting (MDM) outcomes, treatment received and review appointments were collected. We identified 6 ethnicities. To assess patients' SES, we used the Index of Multiple Deprivation score and grouped them into quintiles, with the lowest quintile representing the most deprived. Chi squared, ANOVA and Mann-Whitney tests were performed to evaluate how care process differed in patients with and without anxiety first, and then in patients with and without depression.Results/Outcomes: Out of 2001 patients, one patient had to be excluded for the initial analysis. 145 patients suffered from anxiety. The distribution of patients across ethnic groups and deprivation quintiles showed no significant variation in those with and without anxiety. The analysis did not reveal substantial differences in clinical variables between these groups. The percentage of patients undergoing surgery was higher in the group with anxiety, but this result was only of borderline statistical significance. The second analysis compared between patients with and without depression. There was missing depression data for 8 patients, and thus the analysis was based on 1,993 patients. 269 patients suffered from depression. Main complaint varied significantly between the two groups with anal incontinence more prevalent in depressed patients (p=0.005). The distribution of patients across different ethnic groups and quintiles of deprivation showed no significant variation between depressed and non-depressed patients. Referral from Telephone Assessment Triage Clinic (TTAC) to tests, diagnostic tests, discussion in MDM were all statistically significantly higher in depressed patients (p=0.03, p=0.04, p=0.04). Discussion: This study highlighted depression influence in the patient care process and that patients’ mental health must be taken into consideration to achieve the best treatment. We did not identify significant associations between anxiety, depression, and socioeconomic status (SES) or ethnicity. Future studies with larger patient populations may be warranted to explore potential correlations and develop strategies for targeted emotional support.
