Purpose/Background: There is an increasing incidence of Crohn’s disease (CD) in Kuwait, with evolving management expertise. The aim of the study is to describe the surgical outcomes of ileocolic CD and patterns of recurrence in a consecutive cohort of patients treated within the Kuwait public healthcare system.Methods/Interventions: Following institutional review board approval, all patients (age > 18) who underwent ileocolic resection for CD between November 2011 to May 2022 from a university affiliated government hospital in the state of Kuwait, were included in the study. Demographic, clinical, perioperative characteristics, and postoperative outcomes were included and described.Results/Outcomes: There were ninety-three patients meeting inclusion criteria between the period of 2011 to 2022. The average age at time of surgery was 31.7 (±11.5) years and 57.0% were male. Fifteen percent were smokers at the time of operation. There was a positive family history for Crohns in 24.7% of patients and 11.8% had a prior intestinal resection. The average duration between diagnosis of CD to operative intervention was 10.4 (±6.3) years. There were 51.6% of patients on biologic treatment prior to surgery, 90.3% were elective procedures and 30% were completed laparoscopically. The indications for operative intervention are as follows: 30% stricturing disease, 9.7% fistulizing disease, 3.2% both stricturing and fistulizing, and 8.6% inflammatory. Perianal disease was present in 25.8% of patients. Fourteen patients had a positive margin on their surgical specimen. 32.3% were on biologic treatment post operatively. Overall complications occurred in 16.1% and anastomotic leak occurred in 3.2%. Mean follow up was 34.8 (±35.9) months. During the follow-up period, 19 patients (20.4%) were found to have endoscopic recurrence of CD (Rutgeerts’ score i2 and i4). We found no association between recurrence and sex, family history, previous intestinal resection, smoking, or the presence of perineal disease or extraintestinal manifestations.Conclusion/

Discussion: This study suggests that the surgical management of CD has comparable outcomes to international data with endoscopic recurrence being the most prevalent form of recurrence.
