Purpose/Background: Despite advances in treatment, local recurrence remains a great concern in patients with rectal cancer. Radical surgical resection is considered a treatment option for local recurrence of rectal cancer. The aim of this study was to evaluate oncological outcome for patients with surgical resection for local recurrence of rectal cancer.Methods/Interventions: We retrospectively analyzed the treatment outcomes of 53 patients who underwent surgical resection for local recurrence of rectal cancer in our single center over a 30-year-period. All patients who underwent surgical resection for local recurrence of rectal cancer had no history of neoadjuvant radiotherapy to the primary tumor.Results/Outcomes: Seventy-seven percent (41/53) of patients were diagnosed with local recurrence of rectal cancer within 3 years after primary tumor resection. Surgical procedures for local recurrence of rectal cancer included rectal amputation in 24 cases, tumor resection in 13 cases, Hartmann’s operation in 7 cases, total pelvic exenteration in 6 cases, and low anterior resection in 3 cases. Of those, 21 percent (11/53) of patients had combined resection of the coccyx and/or sacrum. Operating time, blood loss, and postoperative hospital stay (all median values) were 575 minutes (184-1109), 1698 ml (220-5972), and 39 days (8-201). Postoperative complications were observed in 77 percent (41/53) of patients. Of those, 40 percent (21/53) were Clavien-Dindo classification Grade 3a or higher. Seventy-five percent (40/53) of patients underwent curative surgical resection. The 3- and 5-year overall survival rates (OS) were 62 percent and 44 percent for all patients (n = 53), and 71 percent and 47 percent for the curative resection group (n = 40). The 3-year OS of the curative resection group was significantly better than that of the non-curative resection group (no patients in the non-curative group survived for 3 years) (p=0.0009). Of the curative resection group, 55 percent (22/40) of patients had re-recurrence within 3 years. Sixty-four percent (14/22) of patients with re-recurrence had metastatic tumors in multiple organs, including 15 cases in the lungs, 14 cases in the pelvic cavity, 7 cases in the liver, 5 cases in the lymph nodes, 3 cases in the bone, and 2 cases in the brain.Conclusion/

Discussion: Although surgical resection is highly invasive, it increases the chance of cure for the patients with local recurrence of rectal cancer. To further improve the prognosis of patients with local recurrence of rectal cancer, novel treatment strategies are needed in addition to surgical therapy.
