Purpose/Background: The incidence and mortality of colorectal cancer (CRC) has been declining over the last three decades. However, the incidence of early onset CRC (EOCRC) has been increasing over the same period. Several studies are looking at the mechanisms involved in EOCRC. Some authors have showed differential expression of gene expression quantification and transcriptome profiling in these cancers. The aim of this study was to study the differential expression of methylation in CRC among different factors including age, sex, race and stage of disease.Methods/Interventions: The Colon Adenocarcinoma (COAD) dataset of The Cancer Genome Atlas (TCGA) was queried. The search strategy utilized the Illumina Human Methylation 450 platform for data DNA Methylation as the data category and Methylation Beta Values as the data type. The analysis was also done based on the sample type of primary tumor and normal solid tissue. The analysis was done in R as the programming language using the TCGAbiolinks library. The query was downloaded and prepared for the analysis. DNA methylation as well as clinical data was obtained. Age was stratified into < 50 years (EOCRC) and >50 years (LOCRC). The differential expression of methylation beta values was compared for the variables of age, sex, race, sample type and stage. The mean beta values were plotted using boxplots with confidence intervals and p-values.Results/Outcomes: A total of 452 patients in the TCGA-COAD dataset with clinical and genomic data were included. There was a statistically significant difference in DNA methylation beta values between EOCRC and LOCRC (p=0.027). There was also a statistically significant difference in DNA methylation and the stage of cancer at the time of diagnosis (p=0.013). However, there was no statistically significant difference in methylation beta values based on sex (p=0.45), sample type (p=0.57), and race (p=0.45).Conclusion/

Discussion: There was a differential expression of DNA methylation between EOCRC and LOCRC as well as based on stage. This presents a target for studying the mechanisms underlying EOCRC. Further study is needed to look at specific genes for potential targeted treatments based on DNA methylation and have potential application for post treatment surveillance.Figure 1Expression of DNA Methylation by Age, Stage at Diagnosis, Sex, Race, and Sample Type
