Purpose/Background: Colorectal cancer is one of most widespread cancers, as well as the leading cause of cancer related deaths worldwide. Malignant colonic obstruction is especially lethal condition in colorectal malignant disease. There are several studies concerning obstructive colonic cancer (OCC). However, there were few studies which are comparing right and left-sided OCC. Our investigation aimed to compare clinical and pathological parameter, and short-term outcome between right and left-sided OCC after curative resection for colorectal cancer (CRC).Methods/Interventions: A total of 133 patients were analyzed retrospectively, who underwent tumor resection with curative intent for OCC without distant metastasis from January 2016 to February 2022 at a single institution. All patients complained of obstruction symptoms, were diagnosed by X-ray and computed tomography, and patients who passed colonoscopy were excluded. They were classified into right OCC group (n = 38) and left OCC group (n = 95).Results/Outcomes: Decompressive procedure before index surgery was more frequently performed in left-sided OCC (65.8 vs. 96.8%, p < 0.001). Time from bridge procedure to surgery was shorter in right-sided OCC (7.8 day vs. 32.8 days, p = 0.001). Grade of differentiation (Well differentiated + Moderately differentiated/Poorly differentiated + Mucinous adenocarcinoma) was poorer in right-sided OCC (23.7 vs. 8.4 %, p = 0.017). Patient in right OCC group showed higher postoperative pain score (5.5 vs. 4.7, p=0.017). No significant difference was detected in major complication, overall survival rate and disease-free survival rate between both groups.Conclusion/

Discussion: There was no significant difference between right-sided OCC and left-sided OCC in terms of prognosis and complication. Decompressive procedure is more necessary in left-sided OCC and grade of differentiation have association with obstruction in right-sided OCC.
