Purpose/Background: It is estimated the rates of early onset colorectal cancer will continue to increase. Potential risk factors have been investigated; however, there is still limited data regarding long term outcomes and disease characteristics. Research has shown similar long-term outcomes in the early onset colorectal cancer population when compared with late onset disease. Our aim was to investigate differences in disease characteristics and outcomes in patients 50 years old and younger who were diagnosed with colorectal cancer.Methods/Interventions: Our study included patients who underwent colorectal resections between 2015- 2020 in a tertiary referral center. Demographic characteristics, insurance status, disease stage, perioperative variables and long term outcomes were collected. Due to the number of patients in each group, patients in Black, Hispanic, Asian, Native American were analyzed in one group. Patients who are 50 years old and younger were compared with patients who are older than 50 years. These were compared using Fisher’s exact test for univariate analysis and p < 0.05 was considered statistically significant.Results/Outcomes: A total of 753 patients were included in the study, 89 were less than or equal to 50 years old (early-onset) and 664 were greater than 50 years old. Patients in the early onset colorectal cancer group were more likely to have metastatic disease at time of presentation, 24.7% compared to 12.8. (p= 0.0025). However, when comparing the two groups, the greater than 50 year old group had a higher percentage of mortality, 36.3% compared to 14.6%. (p < 0.0001). Multivariate analysis did not associate early onset age with recurrence (p=0.2) Details of Race, disease stage and death are listed in Table 1.Conclusion/

Discussion: Patients with early onset colorectal cancer were more likely to have metastatic disease at the time of presentation, but lower rates of mortality. Early onset colorectal cancer was not independently associated with recurrence based on our data. Further research is required to describe the factors which may explain disease behavior and affect outcomes.Table 1
