Purpose/Background: Postoperative complications like dehydration result in high readmission rates post-ileostomy. Wearable biosensors for continuous remote patient monitoring have shown promise, yet their application in ileostomy patients is understudied. This study utilized a continuous remote patient monitoring (CRPM) solution, encompassing a wearable biosensor with machine learning algorithms, daily electronic patient-reported outcomes (ePROs) for symptoms and stoma output, and a structured escalating protocol to improve post-discharge care in ileostomy patients.Methods/Interventions: A prospective study was undertaken from August 2021 to September 2022. English-speaking patients aged >18 undergoing elective ileostomy creation were included. Participants received study kits with biosensors for continuous physiological data collection and a study smartphone for daily ePROs. The cloud-transmitted data was analyzed using machine learning algorithms to generate clinical and operational alerts. Home health nurses reviewed the platform daily, escalating abnormal findings to the clinical team for prompt intervention.Results/Outcomes: Nine patients, mean age 59.89 (± 15.66) years, completed the study. On average, patients adhered to completing daily ePROs 60% time and stoma output survey 70% time. A total of 90 clinical and 10 operational alerts were generated. One unavoidable readmission occurred due to abscess formation.Conclusion/

Discussion: The program allowed for additional check-ins and early intervention for postoperative complications. The CRPM solution represents an effective patient-centered approach for high-risk surgical ileostomy patients.
