Purpose/Background: We present an interesting case of perforated diverticulitis complicated by recurrent symptoms despite appropriate surgical management due to small bowel foreign body perforation.Methods/Interventions: The patient is a 54-year-old healthy male who initially presented with complicated diverticulitis after failing outpatient therapy. He successfully underwent drainage of abscess with Hartman’s colostomy creation. His initial postoperative course was uncomplicated; however, he presented a few months later with recurrent symptoms. At this time, he had multiple hospitalizations for pelvic abscesses and concern for recurrent diverticulitis of the ostomy.Results/Outcomes: After his third recurrence of symptoms, the patient ultimately underwent surgical exploration and was found to have a segment of small bowel adhered near the ostomy at the abscess cavity site. Within this segment was a foreign body resembling a chicken bone that was deemed the source of the symptomatology.Conclusion/

Discussion: In the review of the literature, perforations related to swallowing foreign bodies are well documented. Foreign body perforation has the potential to mimic a wide variety of gastrointestinal diseases and patient history is often unrevealing, making this a very difficult diagnosis. A high clinical index of suspicion, thorough patient ingestion history, and meticulous radiographic investigation in an acute abdomen of unknown origin is essential.
