Purpose/Background: Previous studies have described cancer health disparities, including colorectal cancer (CRC), in the Texas-Mexico border region. The impact of residing in a border county with port of entry (BC+POE) on CRC survival outcomes has not been reported. Our study compares CRC survival outcomes in patients from BC+POE to border counties without port of entry (BC-POE) along the Texas-Mexico border region.Methods/Interventions: We conducted a retrospective review of adult patients with CRC from the Texas-Mexico border region in the Texas Cancer Registry (2004-2019). All Texas Border Counties (BCs) as defined by La Paz agreement were included. Patients from BC+POE were compared to BC-POE. In-situ cancers were excluded. Chi square tests were used to compare categorical differences between BC groups. Logistic regression analysis was performed to identify factors associated with residing in a BC+POE. Overall survival was estimated using the Kaplan-Meier method and compared among groups using log rank-tests.Results/Outcomes: We identified 8,278 patients with CRC in the Texas-Mexico border region (90% BC+POE). Patients from BC+POE were more likely to be Hispanic (78% vs 68%), uninsured (13% vs 8%), reside in neighborhoods with poverty index greater than 20% (71% vs 66%), and they were less likely to be USA born (63% vs 90%). Age at diagnosis, gender, and tumor site (colon or rectum) were similarly distributed between BC groups. Both BC groups had high rates of metastatic disease (51% vs 54%) followed by localized (37% vs 30%) and regional (12% vs 16%) disease (p < 0.001). In multivariable analysis, BC+POE patients had a higher chance of being Hispanic (OR= 1.7, 95% CI 1.41-1.95), live in a neighborhood with poverty index greater than 20% (OR= 1.2, 95% CI 1.00-1.36), and they had a lower chance of being diagnosed with regional (OR=0.6, 95% CI 0.48-0.75) or metastatic stage CRC (OR= 0.7, 95% CI 0.62-0.86). BC+POE residents had significantly better OS for colon cancer (87% vs 85%, p=0.004) but not for rectal cancer (91% vs 92%, p=0.2).Conclusion/

Discussion: In the Texas-Mexico border region, patients with colon cancer from BC+POE have better survival outcomes compared to BC-POE patients. Patients with CRC from BC+POE were more likely to be Hispanic and live in poor neighborhoods. They were also less likely to be diagnosed with advanced stage disease, which in part could explain the survival differences. However, further studies are needed to identify other contributing factors like having access to cancer care (locally or abroad) to improve CRC outcomes in the Texas-Mexico border region.Demographics by border county group. α Data available from 2013 and onward.
