eP266 - Comparing Outcomes of Robotic Colon to Rectal Surgery in ERAS Protocol

Purpose/Background: Implementation of Enhanced Recovery after Surgery (ERAS ) protocols has been applied more often in colorectal cancer surgery to improve clinical results. This study compares the outcome of patients with colorectal cancer who underwent robotic-assisted surgery versus laparoscopic surgery. We further analyze the patient in colon surgery and rectal surgery. We hypothesize there are different outcomes between colon or rectal surgery under ERAS protocol.Methods/Interventions: The data of this study was collected from a single medical center in Taiwan from Oct. 2019 to Sep. 2023. It's a retrospective cohort study. We enrolled the patients who underwent colorectal surgery either by laparoscopic or robotic Xi platform, all of them are under ERAS protocol. The exclusion criteria are conventional surgery, other major organ combined surgery and emergent operation. This study collected and analysis clinical data including patients’ characteristics, laboratory reports, perioperative profiles, complications, length of stay, histopathologic report, readmission or ER visit within one months, rates of diverting stoma and time to closure it, 28-days mortality rate, long-term prognosis will be discuss.Results/Outcomes: There were 415 patients collected in the robotic-assisted group, with 240 patients undergoing colon surgery, 175 patients having rectal surgery.In the laparoscopic group, 600 patients had been enrolled. After initial analysis, when compared to robotic rectal resection, the patients in the group of robotic colon resection have an earlier discharge, less NG and abdominal drainage tube placement. Shorter time of operation is recorded in the laparoscopic group. There is no statistically difference of leakage rate, 28 days-readmission rate, rate of NG insertion and time to remove.Conclusion/

Discussion: Robotic-assisted surgery can improve the compliance of ERAS protocols and result in better clinical outcomes. Both colon and rectal surgeries benefit from this robotic Xi platform with ERAS. Our study shows no significant difference of complications and length of stay for both groups.
