Purpose/Background: Disparities in access to minimally invasive surgery (MIS) have been described as potential reasons for worse outcomes in minority populations. Yet, access to robotic surgery has likely mitigated MIS access for minority patients. We sought to understand the impact of robotic surgery on access to MIS procedures.Methods/Interventions: We analyzed American College of Surgeons National Surgical Quality Improvement Program (ACS-NSQIP) data from 01/2016 through 12/2021 for all patients treated for colon cancer with elective colectomy. We conducted Poisson Regression models adjusting for surgical specialty, pathologic T and N stage and NSQIP morbidity probability score (based on a logistic regression analysis using the patient's preoperative characteristics).Results/Outcomes: A total of 90,050 patients were included in the analysis with 3,842 Hispanic patients and 6,783 African American patients. During the study period, Hispanic patients were more likely to undergo robotic procedures (adjusted RR 1.25 {95%CI 1.18-1.32}, p < 0.001) but a lower likelihood of receiving laparoscopic surgery (adjusted RR 0.90 {95%CI 0.97-0.93} p < 0.05). In addition, African American patients were also more likely to undergo robotic procedures (adjusted RR 1.14 {95%CI 1.07-1.21} p < 0.001) with a similarly lower likelihood of receiving laparoscopic surgery (adjusted RR 0.95 {95%CI 0.93-0.97} p < 0.001) as compared to White patients.Conclusion/

Discussion: The utilization of robotic surgical techniques in Hispanic and African American patients for colon cancer is expanding significantly. Increased access to robotic surgery may help mitigate some of the outcome differences that minority populations experience after colorectal cancer surgery.
