Purpose/Background: Patients with inflammatory bowel disease (IBD) are at increased risk for developing colorectal cancer (CRC). Despite this, high quality data comparing CRC in patients with and without IBD remains largely limited to cohorts outside the United States. Differences in treatment and outcomes between these two cohorts are poorly understood.Methods/Interventions: We queried a population-based statewide cancer registry of patients diagnosed with colon (including appendiceal), rectal, and anal cancer from 2000-2020 in Indiana Patients were grouped according to the presence or absence of IBD diagnosis by ICD-9 and ICD-10 codes. Patient demographics, tumor characteristics, treatment, and outcomes were compared between groups. The National Cancer Institute Surveillance, Epidemiology, and End Results (SEER) staging was used at diagnosis. IBD diagnosis and colorectal cancer were compared over time. Survival analysis was completed using Kaplan Meier curves.Results/Outcomes: We identified 76,271 patients with CRC. Of these, 501 patients (0.6%) carried a diagnosis of IBD. No differences were noted in gender or race between groups. Patients with IBD were diagnosed with CRC at a younger age (median 63 (IQR 53-74) years vs. 69 (IQR 58-78) years, p < 0.001). Among patients with IBD and CRC, the anatomic site of the tumor was more frequently anal (8% vs. 4%) or rectal (23% vs. 20%) when compared with the non-IBD group, p < 0.001. Colon cancer was less frequent in the IBD group (68% versus 75%), p < 0.001. SEER stage was not different between groups. Rates of treatment with chemo, radiation, and surgery were also similar between groups. The percentage of patients with concurrent IBD and colorectal cancer remained relatively constant over time (Figure 1). Finally, we compared 1-, 3-, and 5-year overall survival between groups and found similar survival rates between patients with and without IBD.Conclusion/

Discussion: We report a large population-based study of over 75,000 patients with CRC. Using this database, we compared patient demographics, tumor characteristics, treatment, and survival between patients with and without IBD. This study provides important baseline data on the role of IBD in CRC on a very large scale. This data serves as a starting point for future studies to further examine the role of IBD in CRC with increasing granularity.Figure 1Percent of colorectal cancer patients with concurrent diagnosis of IBD over time
