Purpose/Background: The optimal time interval (TI) between the two surgical stages of a delayed colo-anal anastomosis (DCAA) has not yet been investigated. Our aim was to assess the possible influence of TI on anastomotic leakage (AL) occurrence in DCAA.Methods/Interventions: This was a multi-center retrospective observational study from 30 tertiary colorectal centers. All patients who underwent DCAA intervention between January 2010 and June 2021 were included. TI was left at the surgeon’s discretion. The main outcome was the occurrence of an AL.Results/Outcomes: 487 patients (male:60%, median age:62.4 years) underwent DCAA. 72% without diverting stoma (DCAA-only) and 28% with a diverting stoma (DCAA-stoma). DCAA-only was mainly indicated after an anterior resection as an alternative to the immediate colo-anal anastomosis protected by a stoma (83%), whereas DCAA-stoma was mostly decided after failure of a previous pelvic surgery (78%). Median TI was 7.0 days IQR[6.0-10.0]. In the entire cohort, TI did not affect the occurrence of AL (p=0.630). TI was longer in DCAA-stoma group (10.0 days IQR[7.0-14.0] vs. 7.0 days IQR[6.0-8.0], p < 0.001). AL risk was not associated with TI among DCAA-only patients (p=0.749) whereas it was among DCAA-stoma patients (p=0.017). A TI≥14 days was identified as the most significant threshold to reduce the risk of AL among DCAA-stoma patients (AL rate: 32% (31/98) with TI < 14 days versus 8% (3/40) with TI≥14 days, p=0.003).Conclusion/

Discussion: Increasing the TI between the two stages of a DCAA does not influence the risk of AL when DCAA is done without diverting stoma. In high-risk patients for whom the DCAA is fashioned with a diverting stoma, a minimal TI of 14 days would reduce the risk of AL.
