Purpose/Background: Colorectal cancer is one of the most common malignancies worldwide, with incidence in younger populations increasing. Its metastatic pattern is typically to the lungs or liver. Here we present a case of a 38 year old male who presented with back pain and was found on CT scan to have lesions in the spine, ribs, and mediastinum with evidence of right colon thickening, concerning for metastatic colon cancer.Methods/Interventions: The patient underwent a complete metastatic workup, which revealed a CEA of 1900 in addition to the aforementioned findings. Colonoscopy was performed, which showed an ulcerated, endoscopically obstructing mass in the ascending colon. Biopsies showed poorly differentiated adenocarcinoma with signet ring and mucinous features, MMR proficient. Percutaneous biopsies of the mediastinal lesions confirmed metastatic colon cancer.Results/Outcomes: The patient underwent semi-emergent palliative radiotherapy to the lumbar spine lesions given severe cord impingement. The patient is currently undergoing systemic therapy with FOLFOX with palliative intent.Conclusion/

Discussion: A presentation of a young patient with widely metastatic colon cancer outside the typical routes of dissemination highlights the need for additional study into the genetics and biology of tumors in this population.CT and MRI images showing right colon thickening, mediastinal lymphadenopathy, and lytic lesions of the lumbar spine and compression of the thoracic cord.
