Purpose/Background: The ERAS protocol promoted multimodal analgesia to reduce post-operative pain and opioid use. There are numerous opioid-sparing analgesia regimens following laparoscopic colorectal surgery, but no gold standard protocol exists. This study aimed to compare the efficacy of xylocaine infusion via surgeon-inserted modified continuous transversus abdominis plane (mcTAP) and laparoscopic tap block (TAP) after laparoscopic colorectal surgery.Methods/Interventions: A single-center retrospective study included consecutive patients who underwent laparoscopic colorectal cancer surgery and received either mcTAP or TAP between January 2021 and January 2023. The mcTAP groups received 0.4% Xylocaine through a mcTAP catheter, while patients in the TAP group received a laparoscopic tap block with 0.25% bupivacaine (10 ml) in the four quadrants of the abdomen after camera port insertion. The primary outcome was the reduction in pain scores at 6, 12, 24, 48, and 72 hours after surgery. Secondary outcomes included opioid consumption in the first 24 hours, the second 24 hours, and the third 24 hours following surgery, recovery outcomes, and 30-day post-operative complications.Results/Outcomes: Thirty-two patients in the mcTAP group and 32 patients in the TAP group were included in the analysis. There were no significant differences in baseline characteristics, tumor staging, and operative procedures between the two groups. Continuous local infusions showed a significant change in pain scores at 48 and 72 hours after surgery (mean difference -1.81 (-2.78, -0.84), p=0.001 vs. -0.71 (-1.63, 0.21), p=0.13, and -2.44 (-3.16, -1.72), p < 0.001 vs. -1.64 (-2.59, -0.69), p = 0.002, respectively). There was no significant difference in opioid consumption, operative time, operative blood loss, or 30-day morbidity between the two groups. However, in the mcTAP group, there was a trend toward receiving more nefopam than in the TAP group.Conclusion/

Discussion: Continuous xylocaine infusion via surgeon-inserted mcTAP had a significant effect on controlling pain at 48 and 72 hours after surgery compared to laparoscopic tap block."Comparison of possible predictive factor for recurrence
