Purpose/Background: Liquid biopsies, which are largely noninvasive, are considered useful for monitoring diseases, such as cancer, and are being studied intensively. Hypermethylation of tumor suppressor genes can lead to cancer development and progression through gene silencing. A number of methylated genes have been identified as biomarkers of prognostic factors and therapeutic efficacy. The present study investigated the significance of circulating tumor DNA (ctDNA) methylation in colorectal cancer (CRC).

Methods/Interventions: A total of 272 patients who underwent radical resection for stage I-III CRC from March 2011 to December 2019 were enrolled in this study. CtDNA was extracted from 2 ml of blood plasma collected before surgery. The methylation levels of tumor suppressor genes (CHFR, SOX11 and CDO1) in ctDNA were analyzed using quantitative methylation-specific polymerase chain reaction (qMSP). The relative methylation value (RMV) was used to evaluate the final quantification of DNA methylation in each gene. We divided patients into two groups according to the RMV and analyzed the recurrence-free survival (RFS) using a Kaplan-Meier curve. In a multivariate analysis, we assessed prognostic factors, including other clinicopathological factors, using the Cox hazard ratio.

Results/Outcomes: There were 37 patients with recurrence in this cohort, with no significant differences noted in sex or tumor location (right or left, colon, or rectum). Among the patients with pStage I-III, those with a higher RMV for SOX11 and CDO1 ctDNA had a significantly higher recurrence rate than those with a lower RMV (p = 0.0002 and 0.007, respectively). Including other clinicopathological factors, lymph node metastasis and a high RMV of SOX11 were identified as independent prognostic factors in the multivariate analysis (hazard ratio [HR] = 2.5; 95% confidence interval [CI] = 1.16-5.36 for lymph node metastasis and HR = 2.37; 95% CI =1.2-4.67 for higher RMV of SOX11). Among the patients with pStage III, those with high RMVs of SOX11 and CDO1 also had a significantly higher recurrence rate than those with low RMVs (p = 0.02 and 0.0011, respectively). In addition, high RMVs of SOX11 and CDO1 were detected as an independent risk factor for recurrence in the multivariate analysis (HR = 2.92; 95% CI = 1.29-6.61 for SOX11 and HR = 2.56; 95% CI =1.08-6.04 for CDO1).

Conclusion/Discussion: Our current study demonstrated that patients with higher methylation levels of SOX11 and CDO1 in ctDNA had significantly higher recurrence rates than those with lower methylation levels, and higher methylation levels of SOX11 and CDO1 were identified as independent risk factors for recurrence in pStage III patients. These results indicate that ctDNA methylation can be a prognostic factor for patients with CRC and can be useful for identifying patients who require particularly careful follow-up after surgery.
