Purpose/Background: High resolution anoscopy (HRA) is an effective screening tool for anal dysplasia. Lesion identification after applying 5% acetic acid and with Lugol’s staining is complex and requires an experienced HRA operator. This video aims to be a primer for lesion identification for HRA providers.

Methods/Interventions: This video reviews best practices for HRA using a 4K camera with multiple levels of magnification. We also describe the nuances of lesion identification after 5% acetic acid application and Lugol’s staining.

Results/Outcomes: Various HRA recordings are reviewed to describe anal dysplasia and demonstrate examples of epithelial surface and patterns, lesion borders, and vascular abnormalities. Lugol’s staining is also reviewed. Examples of high grade lesions are then reviewed in the context of these descriptors.

Conclusion/Discussion: This video reviews conventional descriptions of acetowhite patterns and Lugol’s staining in HRA. This comprehensive yet concise video-based review can be utilized for clinician education in HRA.
