Purpose/Background: The potential oncological benefit of extending of the waiting period between neoadjuvant radiochemotherapy (NRCT) and surgical resection for rectal cancer debated. This study aimed to evaluate the impact of extending this waiting period on the 5-year oncological prognosis of locally advanced rectal adenocarcinoma.

Methods/Interventions: The GRECCAR-6 trial was a phase III, multicenter, randomized study involving patients with mid or lower rectum cT3-4 or TxN+ adenocarcinoma. Patients were randomly assigned to undergo total mesorectal excision (TME) either 7 weeks (7W) or 11 weeks (11W) after NRCT (45-50 Gy with 5-fluorouracil or capecitabine).

Results/Outcomes: Among 265 patients enrolled, 133 were randomized in the 7W group and 132 in the 11W group. After exclusion of metastatic patients, 251 had a rectal resection and 199 (75.3%) had been operated in the exact timeframe of the randomized group (per-protocol analysis).
After a mean follow-up of 59.8 months, 41 (16.2%) patients had died. 5-year overall survival (OS) and disease-free survival (DFS) were 82.8% and 70.8%. 5-year OS was not different between the two groups (intention-to-treat (ITT): 81.7% in W7 group versus 82.4% in W11 group, p=0.855; per-protocol (PP): 79.1% in W7 group versus 83.6% in W11 group, p=0.642), as well as for the 5-year DFS (ITT: 71.8% in W7 group versus 70.1% in W11 group, p=0.726; PP: 64.8% in W7 group versus 69.6% in W11 group, p=0.602). No difference was observed between the two groups for distant or local recurrence (p=0.906 and p=0.533 respectively).

Conclusion/Discussion: Extending the waiting period between NRCT and surgical resection from 7 to 11 weeks does not improve the 5-year oncological prognosis in rectal cancer.
