VM73 - Migration of a Penile Prosthesis Reservoir into the Cecum

Purpose/Background: Erectile dysfunction (ED) is a common condition usually affecting males >40 years with a considerable effect on quality of life.
The three-piece inflatable penile prosthesis (PP) is a widely used treatment modality for ED. It consists of inflatable cylinders inside the shaft of the penis, a fluid reservoir under the abdominal wall, and a pump inside the scrotum.
Potential complications of this procedure include infection, pain, bleeding, mechanical failure and rarely device migration can occur.
We describe here the management of a three-piece inflatable PP reservoir migration into the cecum.

Methods/Interventions: This is a case report of a 73-year-old male patient who presented with a 1-month history of lower abdominal pain and distension. He has a past medical history of type 2 diabetes mellitus and a three-piece inflatable PP placed 8 years ago. Abdominal CT scan showed the PP reservoir migrated into the cecum.
A laparoscopic removal of the migrated PP reservoir was performed.

Results/Outcomes: The video demonstrates the intra-abdominal findings and the surgical technique sequence of removal of the migrated reservoir.
The surgery was without any complication and the patient tolerated the procedure well and his clinical symptoms resolved.

Conclusion/Discussion: Reservoir migration is a rare but recognized complication of inflatable PP insertion. Surgeons and clinicians should be aware of the presentation of such complications and be familiar with surgical approaches to manage them.
