Purpose/Background: Tailgut cysts are a very rare type of congenital cyst that are mainly located in the presacral space. It is believed that tailgut cysts are derived from the remnant of the embryonic hindgut.

Methods/Interventions: We present a 83 year-old female presented with lower back pain for one month. MRI showed well-circumscribed, fluid density pre sacral mass 8x8x6 cm, which corresponds a tailgut duplicate cyst.

Results/Outcomes: The patient underwent complete robotic resection of the mass and post-operative course was unremarkable. Histopathologic examination confirmed a benign cyst.

Conclusion/Discussion: Surgical resection of the presacral cyst is the mainstay of the treatment and the robotic approach is safe and feasible making the dissection of the large lesions easier.
