VM07 - Robotic-Assisted Laparoscopic Perineal Hernia Repair with Mesh Following Abdominoperineal Resection

Purpose/Background: Perineal hernias are an underreported complication of abdominoperineal resections, and the management can be challenging. There are several proposed techniques, such as a natural tissue repair or mesh repair, and the decision can be made to approach the hernia with either a transabdominal or perineal approach.
66 year-old woman presents for surgical evaluation of her perineal hernia. She complains of a perineal bulge that worsens when standing and on exertion.
She has a history of anal squamous cell cancer and eventual abdominal perineal resection performed in 2015 for disease recurrence.
Operative techniques and options are discussed, and patient agrees to proceed with robotic-assisted laparoscopic perineal hernia repair with mesh. This project presents a video vignette of the repair.

Methods/Interventions: Technique for transabdominal repair of perineal hernia using intraperitoneal circular polypropylene-coated mesh.

Results/Outcomes: Successful reduction and repair of perineal hernia, resolution of symptoms.

Conclusion/Discussion: Robotic-assisted laparoscopic perineal hernia repair with intraperitoneal mesh is a proven method for reliable repair and is safe in the elective setting.
