Purpose/Background: High resolution anoscopy (HRA) and treatment of high-grade dysplasia reduces the risk for squamous cell carcinoma in patients living with human immunodeficiency virus (HIV). This video demonstrates the equipment and methods of office-based HRA with fulguration of high grade squamous intraepithelial lesions (HSIL).

Methods/Interventions: 69 year old male with a past medical history of HIV, chronic kidney disease, high risk human papillomavirus, and prior operative fulguration of anal lesions for high grade dysplasia presented for HRA after a screening anal Pap smear revealed atypical squamous cells. Office-based HRA was performed.

Results/Outcomes: After acetic acid and Lugol’s solution application, three areas of suspicious lesions were visualized on HRA. Pathology evaluation of biopsies revealed HSIL in all three areas. The patient was brought back to clinic for fulguration of these lesions. The patient tolerated the procedure well and was discharged home from clinic

Conclusion/Discussion: This video describes the safe and effective application of office-based HRA and fulguration for the treatment of anal dysplasia.
