Purpose/Background: Dr. John Goligher first described the extraperitoneal colostomy in 1958. The technique fell away from interest, but there has been a recent resurgence in popularity to prevent parastomal hernias.

Methods/Interventions: We are presenting a video describing creation of an extraperitoneal end colostomies in laparoscopic abdominoperineal resections through several cases. We demonstrate several techniques to facilitate successful creation of the extraperitoneal colostomy, such as utilization of an EEA sizer for dissection of the tunnel. We also demonstrate one case in which the peritoneum was prematurely entered during creation of the tunnel and how to successfully proceed despite the peritoneal defect.

Results/Outcomes: All patients demonstrated in the cases are currently doing well without parastomal hernia.

Conclusion/Discussion: Dr. Goligher’s method for creating an extraperitoneal colostomy can be safely performed in laparoscopic surgery. Several techniques can be utilized to facilitate successful creation of the extraperitoneal colostomy, and the method can be completed even if the peritoneum is prematurely entered during creation of the tunnel.
